Participant Waiver

All participants must have a signed participant waiver form prior to attending.  You may fill the form out online below and submit it directly, or you may download the document here, sign it, then email it, or bring it to your appointment.

If you are signing for additional minor participants, you may select the option "I am signing this waiver for multiple Minor Participants" and enter the first and last names of each child. A maximum of five minors may be included per submission. Each adult aged 18 or older must complete their own individual online waiver form.  ~ Thank you.
